Investigative Reports

An Investigative Report is for individuals who have had their driver’s licenses suspended or revoked for something other than a DUI. A hearing officer may call for an Investigative Evaluation or Investigative Report. An Investigative Report does not generate a risk classification, but you may still be expected to complete a certain amount of educational classes or treatment.

In order to obtain an Investigative Report, you MUST call us at 217-328-4500 to make a 2-hour appointment. The appointment consists of a test you will take on a Prairie Center computer and an interview with an Evaluator. Based on these two components, the Evaluator will then make a recommendation as to your next steps. It is in your best interest to follow through with the recommendations. Once you successfully complete the recommended next steps, you will receive documents confirming successful completion to bring to your hearing officer along with your Investigative Report.

An Investigative Report is only valid for 6 months.
